Welcome to Potomac Grove Psychiatry. As physicians and psychiatrists, we are committed to helping you get the treatment to help you overcome the psychiatric problems that brought you to this office. Our therapeutic strategy is holistic. We believe that the right combination of psychotherapy, healthy behaviors and, at times, medication can help your body and mind health.

At Potomac Grove Psychiatry, we are aware of the high cost of health care. We encourage you to talk with your doctor/therapist about developing a treatment plan that meets your mental health needs and is affordable for you. It is in your best interest to learn about your mental health insurance benefits and to decide what you can afford to spend for the cost of your care.

Please read the following information about our office procedures, fees, and insurance.

Getting Off to A Smooth Start

Before making your first appointment:

If you plan to use insurance, learn about your mental health benefits first.

Your first appointment:

  • Come 20 minutes early.
  • Bring your completed Personal Profile and your Administrative papers. Note: The doctor needs your completed papers when meeting with you.
  • If the papers are incomplete your appointment time will be shortened while you to complete them.
  • Please bring your picture I.D. and your insurance card. These must be copied for your chart.
  • At the time of your appointment pay your co-pay or full fee for service.
  • Payment may be made by MasterCard, Visa, check or Money Order made out to: Louis E. Kopolow, M.D.P.A.

If PGP does not participate with your insurance plan, we will give you a SuperBill to submit to your insurance.

We encourage you to retain your SuperBill statements. There may be a charge to reprint them again at a later time.

Insurance
If you would like to use insurance benefits toward the cost of your treatment, verify your mental health benefits prior to your first appointment. Don’t guess.
If your insurance plan requires a referral from your Primary Care Physician you must bring your referral to your first appointment in order to be seen.
Payment of Fees

 For insurance plans with which we participate, you pay the required co-pay at the time of your appointment.

For insurance plans with which we do not participate:

  • You must pay the full charge for services rendered at the time of the appointment.
  • We will give you a SuperBill reflecting the services you received and your payment.
  • You may submit this Superbill to insurance, if you choose.

We accept MasterCard, Visa, and checks and MAIL ORDERS made out to Louis E. Kopolow, M.D.P.A. We do not accept cash.

Charges Related to Your Treatment

Some charges for services related to your treatment may not be reimbursable by your insurance. However, you are responsible for payment of services related to your treatment even when not reimbursed by your insurance.
Charges for services related to treatment include and are not limited to:

  • Telephone consultation with patient’s healthcare clinicians.
  • Fee based on level of complexity (low, medium, high).
  • Telephone consultation with patient.
  • Fee based on level of complexity (low, medium, high).
  • Missed appointment or a cancelled appointment made less than 2 business days before appointment.
  • Fee is the charge for the session that was missed.
  • Call to the Pharmacy: fee based on level of complexity (low, high).
  • Returned check. Please check with the office to learn the current charge for returned checks.
